Applying for Service

Reconnection Requirements

·         Full payment of delinquent accounts plus 12% VAT and 10% surcharge.

·         Payment of Reconnection Fee of fifty pesos (P 50) plus 12% VAT.

·         Clearance from Billing Section.

·         Two months advance security deposit if twice disconnected already plus rehab requirements if necessary.

·         Clearance and verification of membership at MSD

·         Required to attend Responsible Membership Course (RMC) or Consultation Dialogue (CD).

·         Signing of pledge and waiver of disconnection.

            Note: No representative allowed applying for reconnection.
