·         Must have signed up application form after reading and thorough understand the provisions therein.

·         Must have paid one hundred pesos (P 100.00) with Official Receipt for membership fee.

·         Must have attended Membership Orientation Course.

·         Must have submitted 2 ID pictures (both spouses).

·         Must have filled up his Index Card requiring specimen signature and thumb mark.

·         Must have been approved by the Board of Directors thru a Board Resolution.
