Here are the requirements:

1. Birth Certificate of any proof of birth
2. Photocopy of Valid Senior Citizen Card with ID#.
3. Other IDs presented (photocopy), any of the following:
   • Passport
   • Driver's License
   • SSS/GSIS ID
   • Voter's ID
   • Postal ID
   • Others ____________
4. 1 pc 1x1 ID Picture
5. Affidavit of two (2) disinterested persons duly notarized and has known the senior citizen for not less than one (1) year
6. Proof of Billing (copy of electric bill issued in the name of the senior citizen)

If applying for a discount through a Representative, pls. submit the following:

1. Authorized Letter - Special Power of Attorney valid for one (1) year from the date of issuance
2. Valid Identification Card of the Representative
